Feedback form template

A short feedback form that produces specific, useful responses. Send it within 24 hours of class.

This form works as a follow-up message, a printed card left on the workstation, or a simple link to a free form tool. Keep it short. Three to five questions. Every question beyond five reduces the response rate significantly.

Send within twenty-four hours of the class, while the experience is fresh.

What to do with the responses

Read every response within a day of receiving it. Look for patterns across multiple sessions rather than reacting to individual responses. After every three to five sessions, review your collected responses and identify:

  • The most common "best part": this is your core. Protect it.
  • The most common "most difficult part": this is your main teaching gap. Address it.
  • The pattern in the rating distribution: if more than one in five responses is "okay" or lower, something structural needs to change.
  • The follow-up interest rate: this tells you how strong your community pipeline is.

Keep a simple log. Date, number of responses, common themes, one thing you will change. Over time, this log becomes the most honest record of how your class is improving.
